Physician behavior
is the subject of intense scrutiny in this
age of rapidly increasing healthcare costs.
The stakeholders in our healthcare system
are many and varied. Employers, healthcare
administrators, quality improvement professionals,
patient groups, pharmaceutical companies
and medical device manufacturers all have
a keen interest in understanding physician
behavior.
The core competency of
Healthcare Performance Consulting is the
ability to assess current behavior of physicians
and other healthcare practitioners, analyze
forces and barriers behind that behavior,
and develop strategic approaches that will
change the behavior; where it is ethical
and medically appropriate to do so. We do
this by taking a strategic approach to changing
behavior. Too often, behavior change “interventions”,
whether they are CME programs or marketing
campaigns, are tactical in nature, and only
act upon one or two aspects of physician
behavior. HPC takes the approach that all
of the major influences on physician behavior
must be explored and considered. A strategic
approach involves a variety of tactics that
act in concert upon each of these major
influences.
The best current theories
on physician learning and change emphasize
the need to examine “readiness-to-change”2.
Change is always a process, and where physicians
are in that process has a bearing on the
strategy used to produce the change. Thus,
an assessment of readiness-to-change is
often a part of the HPC process.
The HPC assessment process
involves a combination of proprietary and
public-domain tools and techniques. A key
component of this process is to involve
“the voice of the learner”,
i.e. to gather information directly from
the physicians themselves. This takes a
variety of forms, including:
- Individual
interviews
- Surveys
- Focus groups
- Chart audits
- Change-Readiness Inventory
- Physician Self-Assessment
- Medical and pharmacy claims data
- Knowledge testing
The HPC consultants then
develop a strategy for changing behavior,
based on what is learned in the assessment
process. This strategy may include a variety
of educational, regulatory, promotional,
and patient-mediated tactics.
