Careful analysis of the data will suggest a change strategy that is most likely to produce the desired outcomes. It is rare that any single intervention, such as an educational program, will produce the desired change. The recommended change strategy may consist of a variety of interventions including patient education, provider education, feedback, reminder systems, incentive changes or policy changes. HPC uses specific tools and techniques that will help in developing the combination of interventions that will most likely produce the desired outcomes.
